In the Dark Realm forest you stumble upon a human female being tortured by goblins. Kill the goblins and talk to her to start the quest.

  1. Find and kill Fizgeez in the other end of the forest.
  2. Go back to the ruined smithy and open the door.
  3. Pick up the ladder.
  4. Enter the ruined smithy to place the ladder.
  5. Climb to the second floor and pick up the shard, Lieutenant Venomfang spawns.
  6. Kill Lieutenant Venomfang to unlock access to the Dark Realm Maze.
  7. Enter the maze and go straight, then right when the road forks. Then left to find an altar of sorts.
  8. Visiting the altar completes the quest and unlocks all other free roam cells in the dark realm.
